I understand that in the event medical intervention is needed, every attempt will be made to immediately contact the persons listed on this form. In the event I cannot be reached in an emergency during class, I hereby give my permission to the physician or dentist selected by the activity leader to hospitalize, to secure x-ray examinations, to secure medical treatment and/or to order an injection, anesthesia, or surgery for my child as deemed necessary. I understand that my insurance coverage will be used as primary coverage in the event medical intervention is needed. I, being the Parent or legal guardian for the student, release Cavod Performing Arts, its staff and instructors, from any and all liability for any injury or illness to the student caused by Cavod Performing Arts, whether or not such injury, illness, or damage was caused by the negligence of Cavod Performing Arts, its staff, instructors, or any other cause. I further agree not to sue Cavod Performing Arts, its staff or instructors for any injuries or illnesses resulting from the student’s participation in any classes or programs conducted by Cavod Performing Arts, whether or not such injury, illness, or damage was caused by their negligence or any other cause. I have carefully read this release and waiver and understand its contents. I am aware that I am releasing certain legal rights that I otherwise may have as a condition of the student’s participation in the classes or programs conducted by Cavod Performing Arts. By checking this box, I give permission for my child to participate in all activities/Cavod classes/camps, to receive emergency treatment, and to be included in any video and/or photograph publications. In addition, I accept that Cavod Performing Arts is not responsible for my student after class/camp is dismissed.
Enrollment is on a first-come, first-served basis. For full classes or camps, a waiting list will be maintained to accommodate other students if and when space becomes available. Classes and camps are limited in size and students are grouped according to age and ability level. In addition, all students must be potty trained and meet the minimum age requirements by Sept 1st to enroll.
A non-refundable $35 fee is due upon registration for school year classes and will be charged to the card on file upon registration. This fee helps to defer the cost of administration, student placement, accounting, and scheduling.
Class and lesson tuition is based on the annual school year, but for your convenience can be divided into 10 equal monthly installments. Class and lesson monthly payments remain the same each month regardless of attendance, school closures, holidays, etc. and all tuition is non-refundable.
We offer risk-free registration: Register and pay. Attend the first class after your registration. Tell us if you don’t want to continue (before the next scheduled class) and we will transfer you to another class, or refund your registration fee and tuition, and unenroll your student. You must attend the first scheduled class after your registration and you must tell us within the 6 days following that first class to be eligible for the refund.
Tuition payments are due the 1st of every month. A late fee of $20 will be levied on any balances not paid by the 10th of each month. Cavod reserves the right to automatically charge the card on file if monthly tuition is not paid by the 10th of the month.
Any student whose account is more than 30 days past due will not be allowed to participate in class.
A $20 fee will be charged for all declined auto-payments. Please call the office if you need to update your credit card on file. No refunds will be granted, regardless of membership type. Tuition should be made payable to Cavod. Payment may be made in person, via the parent portal, or mailed to Cavod, 665 West Main Street, New Holland, PA 17557. We accept checks, cash, Visa, MasterCard and Discover. A $35.00 fee will be charged for all returned checks.
The studio requires a written 30-day notice prior to discontinuing classes or lessons. Monthly memberships will be responsible for 4 weeks tuition from the date the studio receives notification. Annual or Bi-Annual memberships will not be refunded, although the paid tuition can be applied to a different class or camp in the same fiscal year.
I have read and agree with the tuition schedule, school calendar, dress code, and general policies. I understand that there is a costume fee of $70-$95 per class participating in the Dance Concert: $40 deposit per class will be due in November and the remaining balance in December. I understand that all costume and production fees are non-refundable and non-transferrable. I understand that all reminders and communication will be sent through the email provided and I will be responsible for checking my email and responding when appropriate.
All students must arrive to class regularly and on time. You can submit absences in your parent portal to notify the office and teachers of an absence. Please also notify us if you will be missing classes for an extended period due to illness or vacation. Tuition refunds will not be made in the case of absences due to illness or vacation. No more than 9 absences (per class) are allowed throughout the school year in order to perform in the Spring Dance Concert. All students must attend class the last 6 weeks before the Dance Concert. If they do not, the instructor has the right to remove them from the choreographed dance. Classes cancelled due to weather or teacher emergency will be attempted to made up, but due to teacher and studio scheduling, we cannot guarantee a make up.
Private lessons require at least 4 hours-notice from the family for a cancellation to be eligible for a make-up (maximum 2 lessons per year for committed music lessons). If a lesson must be cancelled due to teacher emergency or weather, a make-up opportunity will be offered. Please see Private Music Lessons policy for more specific info on committed music lessons.
It is the responsibility of the parent or adult student to be aware of activities such as productions and dates the school is open and closed. Notices will be posted at the studio and emailed. The general calendar can be found at https://cavod.org/about/calendar/ and hard copies are available at the front desk. It is the parent’s responsibility to regularly check to ensure they are informed. In general, parents are not allowed access into the classroom. Video recording or taking pictures during class is not permitted unless special arrangements have been made. If you would like to sit inside a classroom to observe, please attend our Visitors Week. A hard copy of all policies are available at the front desk.
Students and parents will dress and conduct themselves in an appropriate manner while at Cavod Performing Arts. We will teach each student to work hard and seek to encourage each other. Politeness and generosity are highly valued. Proper respect for the staff and fellow students is expected of all students, parents, and siblings. Every student and parent/guardian should behave in a disciplined, responsible manner both on and off site. The studio reserves the right to suspend or dismiss any student or parent/guardian whose attitude, attendance, or conduct is found to be unsatisfactory. Any student or parent/guardian who disrupts class or approaches a teacher, staff member, another student or parent/guardian with the intent to disrupt, cause dissent, or contravene the policies and procedures of Cavod Performing Arts will be dismissed immediately. Parents wishing to speak to an instructor must first make an appointment with the Director of the School.
Cavod Performing Arts strives to offer a place of fun and learning in a minimally restrictive environment. However, if disciplinary action is required, the following steps will be followed: Steps after each infraction: – The instructor will talk to their student about the behavior. – The Director of the School will be contacted & attempt to talk to the student. – The parent/guardian will be called about the student’s behavior. A meeting between the parent, child, teacher, and School Director may be called. – The Director of the School will contact the parent/guardian informing them of the student’s continued behavioral problems. The student will be suspended from the next class. – The Director will notify the parents of the infraction and that the student will be expelled from the remainder of classes. Depending on the severity of the infraction, Cavod Performing Arts reserves the right to accelerate this process and expel the student without intermediary steps. Should a student be expelled, the student is welcome to re-enroll during the next term. However, if disciplinary problems continue, the student will be permanently expelled and not allowed to re-enroll in the future.
